Please read the Year 1 (2016-2017) application guidelines below carefully before beginning your application. You must complete both Part 1 (online) and Part 2 (in person) in order for your application to be successful. The online application form will be available beginning on Monday, 21st September ’15.

The two parts of the CKY Year 1 application are as follows:

PART 1: Online Application Upload (ONLINE)

Application Upload Week: 21st – 25th September ‘15.

The online application upload requires that you:

•      – complete an online form
•      – select an interview date and time
•      – upload a digital copy of your child’s birth certificate (PDF or JPEG) (4MB)
•      – upload a digital copy of your child’s latest school report (PDF or JPEG) (4MB)
•      – make a note of your application number (VERY IMPORTANT!)
•      – print application form and stick on a recent passport size photo

Please be aware that your application CANNOT BE CHANGED once submitted. 

PART 2: Application Submission (IN PERSON)

Application Submission Week: 24th – 30th September ’15 (9am – 5pm, Mon – Fri, excluding Saturdays, Sunday and public holidays ) 

Address: PLK Choi Kai Yau School, 6 Caldecott Road, Piper’s Hill, Kowloon (IN PERSON)

The second part of the application requires that you submit the following (a minimum of 1 day after your application has been uploaded):

•          – a non-refundable application fee of HK$200
  (cheque made payable to PLK Choi Kai Yau School with your child’s name and application number on the back)

•          – 3 stamped, addressed envelopes ($1.70)
•          – 1 passport size photo of your child
•          – printed application form with photo already attached

Reminders:

• You must wait a minimum of 1 day after your application has been uploaded before completing Part 2.
• You will pick up your child’s interview card when submitting your application.
• Have your application form and application number ready in order to retrieve your application.
